Dr. Stacey Beltz, D.O.
Stacey Beltz, D.O. is a Board Certified Family Practice physician who has been serving our community since 2000. Born in Royal Oak, Michigan, Dr. Beltz attended the University of Michigan in Ann Arbor. She completed her medical degree at Michigan State University College of Osetopathic Medicine. Dr. Beltz completed an internship with family practice emphasis at Mount Clemens General Hospital in 2001 and her residency training at William Beaumont Hospital. Her special interests include women’s health and preventative medicine. Dr. Beltz joined Troy Family Practice, PLLC in 2005.

Ann Forde, N.P.
Ann Forde, M.S., NP-C, CDE is an Adult Nurse Practitioner who has been with our practice since 2004. Ann has over 30 years experience in nursing. She has worked at Beaumont Hospital (Royal Oak & Troy) in labor and delivery, Maternal-Child Health, Pediatrics, Maternal-Fetal-Medicine, and as coordinater for the Outpatient Diabetes Education Program at Beaumont (Troy).
Born in Blue Island, Illinois, Ann attended The University of Michigan, where she received her Master of Science in Community Health Nursing, and her training as an Adult Nurse Practitioner. She received her undergraduate degrees in Nursing at Oakland University and Henry Ford Community College. She is board certified as an Adult Nurse Practitioner, and is also a Certified Diabetes Educator. Her special interests include diabetes care and education, women’s health, and health promotion/illness prevention.
Ann is board certified as an Adult Practitioner by both the American Academy of Nurse Practitioners (AANP) and American Nurses Credentialing Center (ANCC). Certified by the National Certification Board of Diabetes Educators (NCBDE) as a Certified Diabetes Educator (CDE).

Kathrine R. Kranz, PA-C
Kathrine (Kate) Krant is a board certified Physician Assistant who has been in practice since 2009. She was born and raised in Clinton Township, Michigan. Kate attended Grand Valley State University in Grand Rapids, Michigan where she received her Bachelor’s degree in Health Science and minored in Business and Spanish. She continued her education at Grand Valley and graduated from the Masters of Physician Assistant Studies (M.P.A.S) Program in April 2009. She was employed through Oakland University at Graham Health Center from June 2009-May 2012 where she specialized in adolescent and college health. Her special interests include adolescent health, women’s health, and preventative medicine. She joined Troy Family Practice in May 2012.

Brittany David is a board certified Physician Assistant (PA-C) who joined our practice in May of 2017. She was born and raised in Northeast Ohio where she attended undergraduate school at Ohio Northern University and obtained her Bachelor’s Degree in Biology. From there she attended Indiana State University, obtaining her Master’s Degree in Physician Assistant Studies in 2016. Her special interests include adolescent health, women’s health, psychiatry, and geriatric medicine
